# Sildenafil and food: high-fat meals, grapefruit and what the label says

> The Viagra label says sildenafil may be taken with or without food. A high-fat meal slows it down: in the label's data, peak blood levels came about an hour later and 29% lower. Grapefruit is not on the Viagra label, but it can block CYP3A4, the main enzyme that clears sildenafil, and one study found it raised exposure 23%.

**In short:** sildenafil works with or without a meal, and no food is banned on its label. What food changes is speed. A heavy, fatty meal slows how fast sildenafil reaches its peak in the blood, which can make it feel slower to start. Grapefruit is a different question: it does not appear on the Viagra label, but it can block the enzyme the body uses to clear sildenafil, and in a controlled study it raised total exposure to sildenafil, with the authors warning that levels may become less predictable. This page sets out what the labels and studies actually show, where the evidence stops, and when food is worth raising with a prescriber.

## Can you take sildenafil with food?

Yes. Section 2.2 of the Viagra label, titled "Use with Food," says in full that Viagra "may be taken with or without food" ([Viagra prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=0b0be196-0c62-461c-94f4-9a35339b4501), section 2.2). The patient information adds one practical note: after a high-fat meal, the example given is a cheeseburger and french fries, Viagra "may take a little longer to start working." MedlinePlus says the same thing in plain terms ([MedlinePlus, Sildenafil](https://medlineplus.gov/druginfo/meds/a699015.html)).

Timing and dose are set by the label and your prescriber, not by this page. For what sildenafil is and how it is prescribed, see [what is sildenafil](/what-is-sildenafil/).

## Does food change how sildenafil works?

What the label and studies describe is a change in how fast sildenafil gets into the bloodstream: a meal mainly delays and flattens the peak. In the manufacturer's food study of 34 healthy men, a high-fat breakfast delayed the average time to peak by about one hour, lowered the peak level by 29%, and lowered total exposure (the area under the blood-level curve) by 11%. The authors judged these food effects "not considered to be of clinical significance" ([Nichols et al., British Journal of Clinical Pharmacology, 2002](https://pubmed.ncbi.nlm.nih.gov/11879254/)).

That study used healthy volunteers and measured blood levels, not erections. It shows the drug arrives more slowly after a meal; it does not show that a meal makes the medicine fail. How long a dose stays active once it is absorbed is covered in [how long sildenafil lasts](/sildenafil-how-long-does-it-last/).

## What does the label say about high-fat meals?

The label's pharmacology section gives the figures. Taken fasting, sildenafil reaches its highest blood level within 30 to 120 minutes, with a median of 60 minutes. Taken with a high-fat meal, "the rate of absorption is reduced, with a mean delay in Tmax of 60 minutes and a mean reduction in Cmax of 29%" ([Viagra prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=0b0be196-0c62-461c-94f4-9a35339b4501), section 12.3). Tmax is the time to the peak and Cmax is the height of the peak.

## Why is grapefruit mentioned with sildenafil?

Because of one enzyme. The Viagra label says sildenafil is "cleared predominantly by the CYP3A4 (major route) and CYP2C9 (minor route)" liver enzymes, and that only about 41% of an oral dose reaches the bloodstream ([Viagra prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=0b0be196-0c62-461c-94f4-9a35339b4501), section 12.3). The FDA explains that grapefruit juice can block CYP3A4 in the small intestine, so more of an affected drug enters the blood and stays in the body longer, which can mean more side effects ([FDA, Grapefruit Juice and Some Drugs Don't Mix](https://www.fda.gov/consumers/consumer-updates/grapefruit-juice-and-some-drugs-dont-mix)).

The Viagra label itself does not name grapefruit. It does warn that medicines which block CYP3A4, such as ritonavir, ketoconazole, itraconazole, saquinavir and erythromycin, raise sildenafil levels, some of them several-fold ([Viagra prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=0b0be196-0c62-461c-94f4-9a35339b4501), sections 2.4, 7.4 and 12.3). MedlinePlus, under special dietary instructions for sildenafil, says to talk to your doctor about eating grapefruit and drinking grapefruit juice while taking it ([MedlinePlus, Sildenafil](https://medlineplus.gov/druginfo/meds/a699015.html)). Our [sildenafil interactions guide](/sildenafil-drug-interactions/) covers the medicine interactions in full.

## What did the grapefruit study find?

In a randomized crossover study, 24 healthy men took a single dose of sildenafil with grapefruit juice (two glasses, one an hour before and one with the dose) or with water. Grapefruit juice raised total sildenafil exposure by 23% on average and tended to delay the peak, while the height of the peak was essentially unchanged. Variation between individuals was "pronounced" with both grapefruit juice and water ([Jetter et al., Clinical Pharmacology and Therapeutics, 2002](https://pubmed.ncbi.nlm.nih.gov/11823754/)).

The authors concluded that sildenafil levels "may become less predictable with grapefruit juice," and that although patients usually will not be endangered, it seems advisable to avoid the combination. One study in 24 healthy men is a signal, not a rule for everyone. The FDA adds that the amount of CYP3A4 in the gut varies from person to person, so grapefruit can affect people differently on the same drug, and that Seville oranges, pomelos and tangelos might act like grapefruit for medicines it affects ([FDA, Grapefruit Juice and Some Drugs Don't Mix](https://www.fda.gov/consumers/consumer-updates/grapefruit-juice-and-some-drugs-dont-mix)).

## Are there foods to avoid with sildenafil?

The Viagra label does not list any food to avoid. The two food points that come up are a heavy, high-fat meal, which can slow the start, and grapefruit, which MedlinePlus asks you to discuss with your doctor ([MedlinePlus, Sildenafil](https://medlineplus.gov/druginfo/meds/a699015.html)). Alcohol is a separate question with its own label study ([Viagra prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=0b0be196-0c62-461c-94f4-9a35339b4501), section 7.5), covered in [sildenafil and alcohol](/sildenafil-and-alcohol/).

Diet matters for ED in a broader way. The NIDDK says a healthy diet can lower the risk of developing ED and improve ED symptoms, and it suggests limiting foods high in saturated fat (such as red meat and fried foods), full-fat dairy, refined carbohydrates, foods high in sodium and added sugars, packaged foods, and drinks that contain alcohol ([NIDDK, Eating, Diet, and Nutrition for ED](https://www.niddk.nih.gov/health-information/urologic-diseases/erectile-dysfunction/eating-diet-nutrition)). That advice is about the conditions behind ED, such as diabetes, heart disease and obesity, not about a food clashing with the pill. Our [lifestyle and ED guide](/lifestyle-and-ed/) covers diet alongside activity, smoking and sleep.

## Can a big meal make sildenafil seem not to work?

It can make it seem slower. With a peak that arrives about an hour later and sits lower on average after a high-fat meal, the effect may take longer to show ([Viagra prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=0b0be196-0c62-461c-94f4-9a35339b4501), section 12.3 and Patient Information). The label also states that sildenafil at recommended doses has no effect in the absence of sexual stimulation ([Viagra prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=0b0be196-0c62-461c-94f4-9a35339b4501), section 12.1).

If the medicine seems not to work, tell the prescriber, including what you ate, rather than changing the dose yourself. Our guide to [why sildenafil may not work](/sildenafil-not-working/) goes through the common reasons.

## Is tadalafil different with food?

Yes. The Cialis label says tadalafil "may be taken without regard to food" and that "the rate and extent of absorption of tadalafil are not influenced by food" ([Cialis prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=bcd8f8ab-81a2-4891-83db-24a0b0e25895), sections 2.5 and 12.3). That is a real difference from sildenafil, where a high-fat meal slows absorption.

Grapefruit is a shared caution, though. Tadalafil is also cleared by CYP3A4, and the Cialis label names grapefruit juice, alongside erythromycin and itraconazole, among CYP3A4 inhibitors that "would likely increase tadalafil exposure," while noting the specific interaction has not been studied ([Cialis prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=bcd8f8ab-81a2-4891-83db-24a0b0e25895), sections 7.2 and 12.3). For the wider comparison, see [tadalafil vs sildenafil](/tadalafil-vs-sildenafil/).

## When should you talk to a clinician about food and sildenafil?

Raise it before starting if you drink grapefruit juice or eat grapefruit regularly, or if you take any medicine that blocks CYP3A4, such as certain HIV medicines, antifungals or antibiotics, because those combinations can raise sildenafil levels. The FDA's advice for any medicine is to ask your doctor or pharmacist whether grapefruit affects it and to read the patient information that comes with the prescription ([FDA, Grapefruit Juice and Some Drugs Don't Mix](https://www.fda.gov/consumers/consumer-updates/grapefruit-juice-and-some-drugs-dont-mix)). It is also worth mentioning if the medicine seems slow or weak after meals, so the prescriber can review it with you.

Never combine sildenafil with nitrates such as nitroglycerin, or with
"poppers." If you get chest pain, dizziness or nausea during sex, stop and get
medical help right away, and tell the team when you last took sildenafil. An
erection lasting longer than 4 hours, or sudden vision or hearing loss, needs
  urgent care.

These warnings come from the Viagra patient information and Warnings and Precautions ([Viagra prescribing information, DailyMed](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=0b0be196-0c62-461c-94f4-9a35339b4501), sections 5.2 to 5.4 and Patient Information). The NIDDK lists erections lasting longer than 4 hours, and vision or hearing loss after an oral ED medicine, as problems to call a health care professional about right away ([NIDDK, Treatment for ED](https://www.niddk.nih.gov/health-information/urologic-diseases/erectile-dysfunction/treatment)).
